Weeks 1–2
Pain
Calm the pain down.
Swelling management, restoring full straightening, and protecting quadriceps strength, which is the thing that disappears fastest.
What We Treat
Whether or not you have surgery, what you do before and after decides how well the knee holds up. It is fixable, and it starts with a cause-first evaluation, not a guess.

Why It Happens
An ACL injury is a moment, but the result is decided over months. Whether or not you have surgery, the knee loses quadriceps strength quickly, and the control that let the knee give way in the first place does not fix itself.
Going into an operation stronger and better controlled is one of the most reliable ways to come out of it faster, and the work afterwards is what determines whether the knee holds up when you change direction. That decision about surgery is yours and your surgeon’s. The rehab either side is ours.
The Honest Truth
Nobody regrets starting too early. Plenty of people here wish they had started sooner.
How We Fix It
Every plan has an endpoint. Here is what the four phases look like around an ACL injury.
Weeks 1–2
Pain
Swelling management, restoring full straightening, and protecting quadriceps strength, which is the thing that disappears fastest.
Weeks 2–6
Prime
Full bend and straightening, then single-leg control, working inside whatever restrictions your surgeon has set.
Weeks 6–10
Perform
Progressive strengthening to close the gap with the other leg, then hopping, landing and change of direction in the order the knee earns them.
Weeks 10+
Prevent
Side-to-side testing for strength, hop distance and control before you are cleared. A date on a calendar is not a clearance.
Sharon does 1-on-1 PT sessions, and she does a variety of exercises to help strengthen your muscle. I fractured my ankle playing pickleball and was scared to start using that ankle after the injury. She really helped build that connection to the muscle again.

Questions, Answered
Not always, and that decision is yours and your surgeon’s, not ours. What is true either way is that what you do before and after decides how well the knee holds up. Going in stronger and better controlled is the cheapest thing you can do for the outcome.
When your testing says so, not when the calendar does. We test the injured side against the other one for strength, control and confidence, and we tell you exactly what is still missing.
Most patients see a significant reduction in pain within six visits. A full plan for an ACL injury runs through all four phases, Pain, Prime, Perform and Prevent, and you will always know which phase you are in and what is left. That is a plan with an endpoint, not an open-ended string of appointments.
