This policy explains what information this website collects and how it is used. It covers the website only. The way we handle your medical records as a patient is covered separately by our HIPAA Notice of Privacy Practice, which you can request from us or find with the intake paperwork.

Information we collect

If you contact us through this website, request an appointment or ask for a benefits check, we collect the details you enter, such as your name, phone number, email address and what you tell us about your symptoms. We use that information to answer you, to provide the care or information you asked for, and to communicate with you about it.

You do not need to submit anything to browse this site.

Cookies, IP addresses and analytics

Like most websites, this one uses cookies and records IP addresses. We use them to understand which pages people visit, how long they stay and which links they use, so we can improve the site. This is aggregate data and it does not personally identify you.

You can refuse cookies in your browser settings. Some parts of the site may then work less well.

We may also use small tracking images, sometimes called clear gifs or web beacons, in emails to see whether a message was opened. Every email we send includes a way to unsubscribe.

How your information is shared

We do not sell your personal information, and we do not share it with third parties except in the limited circumstances below.

Sensitive information submitted through this site is encrypted in transit using SSL, and we follow generally accepted industry standards to protect what we hold, including firewalls, intrusion detection and encryption.

No method of transmitting or storing data online is completely secure. We use commercially reasonable measures to protect your information but cannot guarantee absolute security.

Links to other sites

This site links to pages we do not control, including our Google profile and our social media accounts. Those sites have their own privacy policies and we are not responsible for their practices.

Children

We do not knowingly collect information from children under 13 through this website. If we learn that we have, we will make reasonable efforts to delete it. We do treat children as patients, and their records are handled under HIPAA with a parent or guardian involved.

Opting out
