Coverage varies, even inside one carrier

Two people can hold cards from the same insurer and have completely different physical therapy benefits. Deductibles, visit limits, copays and whether a referral is needed all change plan to plan.

So rather than guess, we check. Use the free benefits check above and we will verify your specific plan before your first visit and tell you the number, so there is no surprise on the bill afterwards.

You do not need a referral in Illinois

In Illinois you can be evaluated and treated by a physical therapist without a script from a physician. You can call today and be seen without waiting on an appointment with somebody else first.

Some insurance plans still want a referral on file for reimbursement, even though the law does not require one for treatment. If yours does, we will spot it when we check your benefits and help you get it sorted.

If your plan will not cover it

Then you have options, and we will be straight with you about the cost before anything starts. We accept cash pay, and plenty of people choose it deliberately because it avoids visit limits and the paperwork that comes with them.

If you do not have insurance or your insurance does not cover your services, you will need to complete our Health Insurance Non-Coverage Notice form. Ask us or call (224) 858-5161 and we will send it over.

Workers compensation

We treat work injuries. If your employer has set up your care, bring whatever claim information you have and we will work with the adjuster directly. Several of our patients arrived exactly this way.
