Two forms, before your first visit
New patients complete a short set of paperwork before the first appointment. There are two documents: the patient intake form, and our HIPAA Notice of Privacy Practice.
Download both, fill them in at home, and have them ready for your first visit. Ask us or call (224) 858-5161 and we will send them to you.
Why we ask you to do it beforehand
Because your first visit is an hour, and we would rather spend all of it on you than on a clipboard. Having the forms done means the whole appointment is evaluation and treatment.
The intake form covers your history, your current symptoms, what you have already tried and what you are hoping to get back to. That last part matters more than people expect. It is what the plan gets built around.
What else to bring
- Loose, comfortable clothing you can move in
- Any imaging or notes from your doctor about the problem
- Your insurance card, if you are using insurance
- The two completed forms
If you cannot get to the paperwork in time, come anyway. We will sort it out when you arrive. See what to expect for the rest of the first visit.
