Generic Diagnoses and Steroid Shots: Why Physical Therapy Works Better for Shoulder Pain
From the physical therapy team at Maxicare in Carpentersville.
“It is probably bursitis. Here is a cortisone shot. Come back if it still hurts.” If that is roughly how your shoulder appointment went, you are in very familiar company.
The problem with a label
Bursitis, impingement, tendonitis. These describe irritated tissue. They do not tell you why the tissue got irritated, and that second question is the one that decides whether the problem comes back.
Two people can be handed the same label and need completely different treatment, because in one the shoulder blade is not moving and in the other the neck is feeding it.
Where injections actually help
A steroid injection can genuinely reduce inflammation and buy you a window of less pain. Used deliberately, that window is useful: it is easier to restore movement and build strength in a shoulder that is not screaming.
The trouble starts when the injection is the whole plan. Pain comes down, nothing about the mechanics changes, and a few months later you are back where you started, possibly asking for another one.
What a cause-first evaluation looks for
- Which movements and ranges reproduce your pain, and which are clean
- Whether the rotator cuff is weak, irritated, or both
- How the shoulder blade moves when you lift, because it usually does not
- Whether your neck refers pain into the shoulder
- What changed in your training, work or life just before this started
That last one gets missed constantly, and it is often the whole answer.
Then the plan does the work
Hands-on work first, to settle the tissue and free the joint. Then restoring the range you lost. Then loading the rotator cuff properly, which is the part that makes the improvement stick.
It runs through four phases, Pain, Prime, Perform and Prevent, and it has an end date rather than an open tab of appointments.
If you would rather understand your shoulder than manage it indefinitely, that is what an evaluation is for. Call or text us on (224) 858-5161, or request an appointment. You do not need a referral in Illinois, and the first look costs you nothing.
