Fit or Folly? The Risk of Overuse Injuries in Running and Sports

From the physical therapy team at Maxicare in Carpentersville.

Almost nobody gets injured doing something obviously stupid. Most overuse injuries arrive during a period when someone was training well, feeling good, and doing slightly more each week than their tissue could adapt to.

Overuse is an arithmetic problem

Tissue adapts to load, and it adapts more slowly than your enthusiasm grows. When the demand climbs faster than the adaptation, something starts to complain. That is the whole mechanism.

It explains why injuries cluster at the start of a training block, after a race entry, or in the first warm week of spring when everyone suddenly runs further.

The warning signs worth respecting

  • Pain that only turns up after you finish, which is the earliest and cheapest stage to catch
  • Pain during the activity that does not yet slow you down
  • Pain that does slow you down
  • Pain that is there at rest, which is the expensive one

Most people present at stage three. Stage one is a conversation. Stage four is a season.

What actually reduces the risk

  • Increase load gradually. Distance, intensity and frequency, and preferably not all three at once.
  • Get proper gear. Shoes and equipment suited to your actual activity, replaced when they are worn out.
  • Warm up properly. Not a stretch on a wall, an actual gradual build.
  • Strength train. The single most reliable protection against overuse injury in runners.
  • Take the rest days. Adaptation happens on those days, not during the session.
  • Vary it. Doing only one thing loads only one set of tissues.

And get assessed before something forces you to

A comprehensive movement assessment looks at how you actually move and where load is being unevenly distributed. It is far cheaper than the eight weeks off that a stress fracture costs.
